Inheriting 121-Year Legacy of Japan’s Nagumo Clinic, Korea’s Only Apprentice-Style Plastic Surgery Training Site

A unique apprentice-style training program at Seoul’s Eom Nagumo Plastic Surgery is drawing international attention from medical professionals and patients, particularly from the United States, as it represents the only heir to Japan’s 121-year Nagumo Clinic tradition outside of Japan.

“Being evaluated by someone is a great opportunity, but it’s also a truly difficult moment,” says Dr. Nam, who represents the 6th generation successor of the 121-year tradition of Japan’s Nagumo Clinic.

The Art of Axillary Incision: Where Apprenticeship Makes the Difference

What sets Umnagumo Plastic Surgery apart is its approach to revision surgeries. While most clinics prefer inframammary incisions for breast revision surgeries due to easier shape control, the Nagumo technique insists on axillary (armpit) incisions that leave no visible scars.

“The difficulty level is higher, but we can achieve beautiful breast shapes even through axillary incisions. This is the true differentiator of the 121-year Nagumo surgical technique,” Dr. Nam explains.

Mastering the Fundamentals: Starting with Grip Technique

Dr. Nam recalls his most memorable training experience – surprisingly, it was learning the proper “grip technique.” Despite having handled surgical instruments more than chopsticks since beginning his plastic surgery career in 2017, he was severely corrected when first demonstrating his grip before Dr. Eom.

“Everything in plastic surgery is manual work. The angle at which you hold instruments, how you apply pressure with your fingertips – accurate grip technique is where the fundamentals begin. I relearned from scratch what no one had ever taught me before.”

Approaching Surgery Like Sports

Dr. Nam compares surgery to sports. The night before surgery, he studies patient photos to plan the design, running mental simulations and preparing for variables. During surgery, he focuses on minimizing bleeding and preserving normal anatomical structures as much as possible.

“After surgery, I always document the procedure that same day. I note what was lacking and what went well, and photograph each patient’s progress under identical lighting and angles to observe changes.”

Beyond Mathematical Formulas: The Realm of Intuition

His mentor’s words, “Surgery is not mathematics,” provided Dr. Nam with profound insight. Although he naturally prefers mathematical calculations, unexpected variables always arise in the operating room.

“The ability to intuitively assess and adapt to situations based on fundamentals – this is a realm that cannot be learned from books or theory. No matter how brilliant the mind, if the hands cannot follow, you cannot create results.”

The Journey from 70% to 100%

Dr. Nam admits that when he first joined the clinic, he aimed for 70% quality surgeries. Now, he pursues cleaner, more perfect procedures. The most challenging aspect was abandoning existing habits and methods to start fresh from a blank slate.
